Captain America Sam Wilson and the new Falcon, Joaquín Torres, are dispatched by newly elected United States President Thaddeus Ross on a high-priority mission to Oaxaca, Mexico, where the terrorist organization SERPENT is attempting to sell a cache of adamantium recovered from Celestial Island. Sixteen years after the controversial Duel of Harlem, Ross has built his presidency on promises of unity and cooperation against global threats. During the operation, Captain America storms the Catedral de Santo Ramos to rescue hostages while battling SERPENT leader Seth Voelker, better known as Sidewinder. Sidewinder tries to force the captives to reveal the identity of SERPENT’s mysterious buyer, but Captain America intervenes before he succeeds. Meanwhile, Falcon intercepts SERPENT’s escape convoy, secures the stolen adamantium canister, and returns it to American custody as Captain America defeats the deadly mercenary Copperhead.
Following the successful mission, Torres expresses his desire to become a better hero and asks Wilson to train him. Wilson takes him to Baltimore to visit Isaiah Bradley, the forgotten super soldier whose painful history with the U.S. government still leaves him deeply distrustful of those in power. While they train together, Wilson receives a phone call from President Ross inviting him to a diplomatic summit at the White House. Bradley admits he is uncomfortable with Wilson working alongside Ross, fearing the new president has not truly changed despite his promises. Wilson reassures Bradley that he intends to protect the ideals of Captain America rather than serve any politician, and reveals that Ross has also invited Bradley to attend the summit. Though reluctant, Bradley accepts the invitation.
Elsewhere, Ross quietly visits the grave of his late wife and confides in his Head of Security, Leila Taylor, that his estranged daughter Betty still refuses to believe he has become a better man. Ross knows the success of his presidency depends upon proving his sincerity during his first hundred days in office. At the White House, Wilson meets Ross privately in the Blue Room, where the president reveals his intention to rebuild the Avengers, believing the world once again needs Earth’s mightiest heroes despite his previous support of the Sokovia Accords. During the international summit, Ross unveils plans for the Ozaki-Ross Accords, a treaty designed to regulate the mining and distribution of adamantium recovered from Celestial Island. Before the agreement can be finalized, the gathering erupts into chaos when Isaiah Bradley suddenly attempts to assassinate Ross. Although Bradley appears completely unaware of his own actions, he is arrested after fleeing the scene, leaving Wilson convinced something far more sinister is responsible.
Determined to prove Bradley’s innocence, Wilson confronts Ross and his security advisor, Ruth Bat-Seraph, demanding permission to investigate the attack. Ross refuses and orders him to stay away from the case, but Wilson ignores the command. After visiting Bradley at Joint Base Anacostia-Bolling, Bradley desperately insists he has no memory of trying to kill the president and begs Wilson not to become involved. Wilson refuses to abandon him. Torres examines White House surveillance footage and discovers that the assassins’ phones emitted synchronized flashing lights moments before the attack, suggesting they were victims of sophisticated mind control. Bat-Seraph independently reaches the same conclusion.
Wilson soon realizes the unknown buyer who hired SERPENT is orchestrating events from the shadows. Before he can pursue the lead, Sidewinder ambushes him, having been promised double payment to eliminate Captain America. Following a fierce battle, Wilson defeats the mercenary, who narrowly escapes capture. Immediately afterward, Wilson receives a call from the mysterious mastermind, who congratulates him for surviving and ominously promises that far worse is still to come. Meanwhile, Ross works behind the scenes to reassure world leaders that the treaty negotiations will continue despite the assassination attempt.
Tracing Sidewinder’s communications, Wilson and Torres travel to Camp Echo One, a secret military prison hidden in Green Bank, West Virginia. At the same time, Bat-Seraph fails to prevent Sergeant Frank Payumo, one of the mind-controlled assassins, from killing the remaining conspirators before taking his own life. During Ross’ flight to Japan, he finally realizes that the mastermind manipulating events is Samuel Sterns, the brilliant scientist he secretly imprisoned following the Duel of Harlem. Sterns coldly vows to destroy Ross’s presidency and erase any chance of redemption.
Inside Camp Echo One, Wilson and Torres discover a hidden laboratory where Wilson finds medication prescribed exclusively to Ross. Their investigation leads them directly to Sterns, whose exposure to Bruce Banner’s irradiated blood years earlier transformed his brain and granted him extraordinary intelligence. Sterns reveals that Ross exploited his genius for sixteen years, forcing him to solve countless political and military crises while repeatedly denying the promise of freedom. He insists he never intended to assassinate Ross, but instead seeks to expose his hypocrisy. Escaping confinement, Sterns unleashes brainwashed soldiers against Wilson and Torres. With Bat-Seraph’s assistance, the heroes defeat the attackers, but Sterns slips away once more. Their next stop becomes a military training facility in Norfolk, Virginia, where Sidewinder is now imprisoned.
While Ross sails toward Celestial Island aboard the USS Milius to secure America’s claim to the adamantium before Japan can do the same, Sidewinder reveals Sterns’ true objective: provoking a military conflict between the United States and Japan while permanently destroying Ross’s public reputation. Wilson’s longtime military friend Dennis Dunphy warns him that Japanese warships have already converged near the island. Captain America and Falcon immediately launch toward the growing international crisis.
Aboard the USS Milius, Wilson confronts Ross about the medication discovered at Camp Echo One. Ross finally admits the truth: Sterns developed pills that prolonged his failing life, and Ross kept Sterns imprisoned to guarantee continued treatment, hoping to live long enough to reconcile with Betty. Before Ross can make amends, Sterns activates his final plan. Two Navy fighter pilots fall under mind control, and one launches an attack against Japanese aircraft. Captain America and Falcon desperately intervene, attempting both to stop the rogue pilots and prevent the Japanese military from retaliating. During the battle, Sterns manipulates Ross emotionally while revealing that the life-saving pills were secretly saturated with gamma radiation. Falcon is critically injured after being shot down by one of the possessed pilots, but Captain America succeeds in neutralizing both aircraft before a full-scale war erupts.
Torres is rushed to Walter Reed National Military Medical Center, where Wilson receives encouragement from Bucky Barnes as he struggles with the burden of leadership. Ross prepares to address the nation during a press conference in Washington before meeting Betty. However, Sterns hijacks the broadcast after preventing Dennis Dunphy from warning Wilson about the gamma poisoning. Sterns publicly exposes Ross’s secret dealings and releases evidence of their years-long conspiracy. Surrounded by reporters and consumed by guilt, anger, and the accumulated gamma radiation within his body, Ross loses control and transforms into the monstrous Red Hulk. In a blind rage, he tears through the White House, reducing much of the historic building to rubble.
Captain America confronts Red Hulk amid the devastation, drawing him away from civilians and into East Potomac Park. Knowing brute force alone cannot win the battle, Wilson repeatedly reminds Ross of his love for Betty and his sincere desire to become a better man. Even after Red Hulk smashes through Captain America’s vibranium wings and devastates the surrounding area, Wilson refuses to stop believing in Ross’s humanity. At last, Ross accepts that he truly wants to change, and Wilson’s unwavering faith breaks through the monster’s fury. The Red Hulk calms, reverts to human form, and peacefully surrenders.
In the aftermath, Isaiah Bradley is officially cleared of all charges after evidence proves he was another victim of Sterns’ mind-control technology. The Ozaki-Ross Accords are successfully ratified by the United Nations, ensuring international cooperation over adamantium resources. Ross resigns from the presidency and willingly accepts imprisonment at the Raft for his crimes. Wilson visits the disgraced former president, explaining that despite everything, people must still strive to see the good in one another. He then quietly leaves so Betty can finally reconcile with her father.
At Walter Reed, Torres apologizes for allowing his inexperience to endanger the mission. Wilson admits that carrying the Captain America shield comes with enormous pressure but assures his protégé that he has the potential to become a true hero. Encouraged by Wilson’s confidence, Torres eagerly looks forward to returning to action once he recovers, joking that Wakanda should give his Falcon suit a major upgrade.
Later, Wilson returns to the Raft to confront the imprisoned Samuel Sterns one final time. Confident that hope and compassion ultimately prevailed, Wilson remarks that he has won their wager over whether Ross could truly change. Sterns dismisses the victory, warning that their conflict was only the beginning. He cryptically reveals that an even greater danger is approaching from beyond their universe—a threat from other worlds that will test everything Captain America is willing to sacrifice to protect. Wilson leaves the prison knowing that an even larger battle lies ahead.
